The WYhealth Care Management Program is a benefit our Medicaid members have through Wyoming Medicaid and can help those who need some extra support to meet their health goals, understand their health condition(s), and get help with other needs like food or transportation.
The WYhealth Program has registered nurse care managers who would love to connect with you/your patient! Once someone agrees to participate, a registered nurse care manager will reach out via telephone to work with the individual each month on their health goals.
